Japanese scientists have created an injectable protein therapy that helped cats with advanced kidney disease survive far longer in early trials. Here is what the treatment really does, and whether it can truly push feline lifespans towards 30 years.Cats almost always die of kidney disease because they cannot activate a waste-clearing protein other mammals use freely.For most cat owners, the vet's words land like a stone: kidney disease. Now a team of scientists in Japan has developed an injectable protein therapy, filed for approval in April 2026, that kept cats with advanced kidney disease alive far longer in early trials, and whose lead researcher believes it could one day help cats live up to 30 years.That last figure is a hope, not yet a fact, and we will come to why. But the science underneath it is real, published in respected journals, and genuinely new. For the first time, a treatment is trying to fix the actual cause of feline kidney failure rather than simply slowing it down.WHY DOES KIDNEY DISEASE KILL SO MANY CATS?Chronic kidney disease, or CKD, is the slow, steady loss of the kidney's ability to filter waste from the blood. It is the single biggest killer of older cats.By some estimates, it affects around 30 to 40 per cent of cats over 10 years old, and up to roughly 80 per cent of those over 15. When it fails, poisons that should leave the body in urine instead build up in the blood, bringing loss of appetite, weight loss, dehydration and, eventually, death.Until now, vets could only slow the decline with special diets, fluids and medicines. Nothing repaired the damage.WHAT IS THE AIM PROTEIN, AND WHY DOES IT MATTER? The story turns on a protein called AIM, short for Apoptosis Inhibitor of Macrophage, discovered by Dr Toru Miyazaki, now director of the Institute for AIM Medicine, a non-profit research organisation in Japan.Think of AIM as a cleanup tag. Inside the body, cells are constantly dying and leaving behind debris. AIM works like a sticky label: it attaches to that rubbish and flags it so the immune system’s cleaners, called macrophages, can come and clear it away. (Photo: Radifah Kabir) In healthy animals, this keeps the kidney's tiny drainage tubes, the tubules, from clogging up.In most mammals, including humans, dogs and mice, AIM does this job well. In cats, something goes wrong.WHY ARE CATS SO UNIQUELY VULNERABLE?This is the discovery that started it all, published in Scientific Reports in 2016.In the blood, AIM normally travels stuck to a large antibody called IgM, a kind of parking spot that keeps it inactive until needed. When injury strikes, AIM is meant to let go of IgM and rush to the damaged kidney. AIM works like a cleanup tag, labelling cellular waste so the cat’s immune system can flush it out and keep the kidney's tiny tubules clear. (Photo: Radifah Kabir) Feline AIM cannot let go. The researchers found that in cats, AIM grips IgM about 1,000 times more tightly than it does in mice, because the cat version of the protein carries an unusual cluster of positively charged building blocks that acts like superglue.So feline AIM stays stuck in the bloodstream, never reaches the kidney, and the debris is left to pile up. Over years, the tubules silently clog, inflammation sets in, scarring spreads, and the kidney fails. It is a flaw written into cat biology itself.HOW DOES THE NEW INJECTION ACTUALLY WORK?Dr Miyazaki's team reasoned simply: if the cat's own AIM will not work, supply working AIM from outside.They engineered a laboratory-made version, called recombinant AIM, that is already active and does not stay locked to IgM. In cats, AIM grips the IgM antibody about 1,000 times more tightly than in mice, leaving it trapped in the blood and unable to reach the kidney. (Photo: Radifah Kabir) Injected into the bloodstream, this free AIM can travel straight to the injured kidney, coat the accumulated waste, and signal the macrophage cleaners to flush it out before permanent scarring takes hold.In the studied protocol, cats received the injection roughly once every two weeks. Rather than masking symptoms, it aims to switch the kidney's own cleaning system back on.WHAT DID THE CLINICAL TRIAL REALLY SHOW?Here is where careful reading matters. The results, published in The Veterinary Journal in February 2026, came from an exploratory, non-pivotal study, which means an early, promising look rather than final proof.Researchers screened 216 cats with CKD across 13 private veterinary hospitals between 2019 and 2024, then treated a smaller group who already had advanced disease. (Photo: Radifah Kabir) The difference was striking. After 360 days, roughly 80 to 83 per cent of treated cats were still alive, compared with about 20 per cent of untreated cats. Treatment also steadied key markers of kidney health and reduced the toxic by-products that poison the blood.For a disease that usually marches only in one direction, that is a remarkable result. But it was a small, early study of cats who were already ill, and it did not regrow damaged tissue. It halted the slide, it did not reverse it.CAN CATS TRULY LIVE UP TO 30 YEARS?This is the headline everyone has seen, and it needs honesty.The average pet cat lives around 15 years, with kidney failure a major reason lives are cut short. Dr Miyazaki's reasoning is that if you remove the biggest killer, especially by treating cats early, many could live far longer, perhaps approaching 30. It is a logical hope, and a lovely one. After 360 days, around 80 per cent of treated cats were alive, against roughly 20 per cent of untreated cats with advanced disease. (Photo: Radifah Kabir) It is also, for now, only a projection. No study has followed treated cats for decades. Other diseases, from cancer to heart trouble, will still take their toll. So the 30-year figure belongs in the column marked optimism, not evidence.The proven claim is narrower and still exciting: in cats already facing advanced kidney disease, this therapy helped many survive a year when most would not have.COULD THIS ONE DAY HELP HUMANS TOO?Possibly, though that lies further down the road. Humans carry AIM as well, and it plays the same waste-clearing role. Scientists are exploring whether adjusting AIM could help human kidney disease and other conditions where the body struggles to clear cellular rubbish.The Institute for AIM Medicine has said human trials are a longer-term goal, once the feline programme matures. If it clears that hurdle, it would be the first therapy to treat the root of feline kidney disease rather than just its symptoms.For millions of cat owners who have watched a beloved companion fade, that alone would be extraordinary.
