A claim requires documentation of a qualifying diagnosis — primary lung cancer, squamous-cell throat cancer, or severe emphysema or COPD at GOLD Grade III or IV — made between March 8, 2015, and March 8, 2019. For many claimants, that means tracking down records from clinics, hospitals, and specialists they may have seen a decade ago, some of which have since closed, merged, or changed systems.That’s the work Medchart does. Claimants provide their information and sign a consent authorization.
