You survived COVID, the flu or H1N1 once, so you may assume your body knows how to fight it the next time. But doctors say immune memory is more complicated than simply remembering or forgetting a virus.Why can COVID, flu and H1N1 infect you again even after your body has encountered them before?You thought you had already been through the worst. The fever, body aches, sore throat, cough, and those anxious days of wondering whether COVID, flu, or H1N1 would get worse. You recovered, returned to your routine and assumed your immune system would protect you if the same virus came back.But doctors say that assumption may not always hold, especially for people with underlying health conditions.COVID and influenza can infect you again even after a previous infection. While these infections are not generally life-threatening for otherwise healthy people, those with comorbidities, people undergoing chemotherapy, and pregnant women can face a higher risk of complications, says infectious disease expert Dr Ishwar Gilada.There is another reason not to ignore flu symptoms. Dr Gilada says antiviral treatment with oseltamivir works best when started within 48 hours of the first symptoms. For people at higher risk of complications, seeking medical advice early can therefore be important. So why does the body allow a virus it has already fought once to enter again?The answer is not that your immune system has completely forgotten. It is that immunity is more complicated than simply being “immune” or “not immune”.Months or even years after an infection, your immune system may still retain memory of the virus. But antibody levels can decline, protection in the nose and upper airways can weaken, and viruses such as influenza and SARS-CoV-2 can change over time.That combination can leave the door open for reinfection.And while a second infection may be mild for one person, it can become a bigger concern for someone whose age, pregnancy, underlying illness or treatment increases their risk of respiratory complications.Dr Ishwar Gilada, Consultant in HIV, STDs and Infectious Diseases at Unison Medicare & Research Centre, Mumbai, and Secretary General of People's Health Organisation India, explains that immunity does not provide the same level of protection against infection and severe disease.“The vaccine offers two different types of protection. The protection against infection and protection against severe disease. And they can be at different levels with different infections,” he says.That distinction is important because not getting infected and not getting seriously ill are not the same thing.YOUR IMMUNE SYSTEM DOES NOT SIMPLY FORGET THE VIRUSAfter an infection, your immune system creates a memory of the pathogen. Memory B cells and T cells remain in the body and can respond if they encounter the virus again. Antibodies also play an important role, particularly in the early stages of an infection.But antibody levels do not remain at the same level forever.“After infection, the body retains memory B cells and T cells, but the antibody level, especially those in the nose and upper airway, declines over time,” explains Dr Gilada.This is where the confusion often begins.If antibodies are present at the place where the virus first enters the body, they can potentially stop the virus before it gets established. But if those antibody levels have fallen, the virus may get an opportunity to infect cells before the deeper immune response is fully activated. Think of it as the difference between having a security guard standing at the entrance and having an emergency team inside the building.The security guard may stop an intruder immediately. If the guard is no longer there, the emergency team inside can still respond, but it may take longer.That is broadly what can happen with respiratory viruses.THE NOSE MAY BE THE WEAK LINKRespiratory viruses such as influenza and SARS-CoV 2 typically enter through the nose and upper respiratory tract. This means immunity at these surfaces, known as mucosal immunity, can be particularly important in preventing infection.Dr Bobby Bhalotra, Vice Chairperson of Chest Medicine at Sir Ganga Ram Hospital, explains that this local immunity can fade relatively quickly.“The mucosa where the virus is going to attack, like the nasal mucosa, the bronchial mucosa, forgets the virus because the memory of those mucosal cells which prevent attack by the virus or which prevent viral multiplication also fades away very, very soon,” he says.This does not mean the entire immune system has forgotten the virus.Rather, the protection at the exact place where the virus first arrives may no longer be strong enough to prevent infection. Dr Bhalotra says, “It is wrong to say that the virus memory is lost from the body, but the truth is that this much memory is unable to prevent future infections.” That distinction is perhaps the most important part of understanding reinfection.COVID CAN CHANGE ENOUGH TO FOOL OLD ANTIBODIESThere is another problem. The virus itself does not necessarily remain the same. SARS-CoV-2 continues to evolve. New variants can carry changes that make existing antibodies less effective at recognising and binding to the virus.This is one reason why someone who has had COVID before can still become infected with a newer variant. The same principle explains why protection against infection can vary as the virus changes.In other words, your immune system may still have the old photograph of the virus, while the virus has changed some of its features. The immune response is therefore not useless. It may simply be less effective at completely blocking the new version at the point of entry.FLU HAS ANOTHER TRICK: ANTIGENIC DRIFTInfluenza viruses are particularly good at changing over time. They undergo a process called antigenic drift, in which small genetic changes accumulate in the virus.As these changes build up, the virus can become sufficiently different from the strain that caused your previous infection that your existing antibodies may not recognise it as effectively. This is also one reason influenza vaccinations are updated regularly.So, having had H1N1 does not necessarily mean you have permanent protection against every future influenza virus you encounter. Your immune system may recognise parts of the virus, but that recognition does not always translate into complete protection against infection.THEN WHAT IS THE POINT OF PREVIOUS INFECTION?This is where the story becomes much more reassuring. A previous infection can still provide valuable protection, particularly against severe illness.Even when antibodies are not strong enough to completely prevent infection, memory B cells and T cells can respond more rapidly than they would during a first-ever exposure.That faster response can help control viral replication and reduce the risk of serious complications.“Immunity is not an on-off switch like an electricity button. The protection against catching the virus may fade, but the immune memory that protects us from severe disease generally lasts much longer,” says Dr Gilada.This is why getting infected again does not automatically mean that the second infection will be as dangerous as the first.For many otherwise healthy people, COVID and seasonal flu are not necessarily life-threatening. However, the risk of complications can be significantly higher among people with underlying health conditions and other vulnerable groups.Dr Gilada says people with comorbidities, those undergoing chemotherapy and pregnant women can be at higher risk of COVID or flu-related complications.Age and the overall health of the individual can also influence how seriously a respiratory infection affects the body.WHY VACCINATION CAN STILL MATTER AFTER INFECTIONIf you have already had COVID or the flu, it is natural to wonder whether a vaccination is still necessary. The answer depends on the specific infection, vaccine recommendations, your age, health status and current public health guidance.But the basic immunological principle remains the same: vaccination is not only about preventing every infection.It can also help strengthen protection against severe disease. With respiratory viruses that continue to evolve, protection can change over time. This is why revaccination or updated vaccination may be recommended for certain infections and groups.Dr Gilada says annual revaccination is advisable for flu because influenza viruses continually undergo antigenic changes.IF YOU GET FLU, TIMING CAN MATTERThere is also an important lesson in what happens after infection. For people at higher risk of influenza complications, antiviral treatment may be recommended by a doctor.Dr Gilada points out that for flu, oseltamivir works best when started within 48 hours of the first symptoms, making early medical consultation particularly important for people who are vulnerable or develop significant symptoms.This does not mean everyone with a cough or fever should immediately take an antiviral. Treatment decisions should be made by a doctor based on symptoms, risk factors and the likelihood of influenza.SO, HAS YOUR BODY FORGOTTEN COVID OR H1N1?No. Your immune system is not a computer that either stores or deletes a virus permanently. After infection, several layers of immune memory can remain. But different parts of that protection behave differently.Antibody levels can decline.Mucosal immunity can fade.The virus can evolve.And influenza viruses can continually change through antigenic drift. That combination means the protection that once prevented infection may not remain strong enough forever. But the immune system may still remember enough to respond faster and reduce the chances of severe disease.So the next time you hear someone say, “I already had COVID, so I cannot get it again,” or “I had H1N1 once, so I am immune for life,” the science is more complicated.You can get infected again. But that does not mean your previous infection was pointless. Your immune system may not be able to keep every virus out forever.
