In anticipation of the joint ACTRIMS-ECTRIMS 2026 meeting, Shiv Saidha, MD, discusses results from the phase 3 REMODEL-1 and REMODEL-2 trials of the oral Bruton tyrosine kinase inhibitor remibrutinib in relapsing-remitting multiple sclerosis. Top-line findings showed that remibrutinib significantly reduced the annualized relapse rates vs teriflunomide, met secondary MRI lesion endpoints, and showed a trend toward delayed 3-month confirmed disability progression and nominal statistical significance for 6-month confirmed disability progression. He notes that interpretation will depend on the full dataset, including teriflunomide-arm performance, absolute disability-progression event counts and effect sizes, and detailed safety data clarifying the reported lack of a liver safety signal. He also anticipates analyses of fenebrutinib. Fenebrutinib previously met the primary annualized relapse-rate endpoint vs teriflunomide in the phase 3 FENhance 1 and 2 relapsing-MS trials and was noninferior to ocrelizumab in the phase 3 FENtrepid trial in primary progressive MS. In the late-breaking session, investigators are expected to present fenebrutinib’s effects on paramagnetic rim lesions and slowly expanding lesions, MRI markers of chronic active inflammation, across FENhance 1, FENhance 2, and FENtrepid.
